Philly Streets - Dermatologist Interview (Dr. Cartwright)
🕑 Added 2023-12-18 21:51:08 +0000 UTCHey guys,
Here's our complete, bonus interview with Dr. Martina Cartwright, a dermatologist who is on the frontier of studying the effects of Xylazine (Tranq) on the body. This is my first time interviewing a doctor, so I'm stoked!
Also, I did this because you kind Patrons let me know that my inferences about Necrotizing fasciitis where a little off base, so thanks for keeping the journalism blade sharp!
Much love

Cool to see you go back and interview a professional after hearing out criticism
Seth Young
Love the sweat pants lady.
Grace Taylor
I work in drug research in SF and we actually aren't really seeing xylazine/ tranq in the supply here yet (per street-based drug checking services).
Abiel Gallagher-Reyes
She’s the kingpin
Samuel Alia
This is depressing, though her work as an individual brings out the importance of her work. Dr. Cartwright, thank you for pointing this out. From a medical point of view.

I am catching up with all the documentaries as I am a new Patreon and I love this more in depth/serious talks with experts. I saw the chat with Professor Gavin too about Israel-Palestine… Well done guys, very happy to see how you have been evolving since the All Gas No Brakes days until today. Keeping it fun and random but serious on topics no other media would. Please keep it up! ❤️🙏

Did this problem disappear from 2016-2020 and magically reappear when Biden was elected? I don’t think so. America’s drug crisis transcends presidencies
Levi Fallstrom
No euphoria at all with it. It's for pure escape from reality. Heroin and Fentanyl provide a euphoric rush. Nothing like this stuff.

Yeah to say someone can't/shouldn't be fact-checked because they have a degree is ridiculous. American's blind faith in medical professionals is why people will take anything their doctor prescribes without a second thought. That mentality/ignorance is great for big pharma though. She refers to xylazine as being like ketamine and she also says it rivals heroin. Xylazine doesn't affect the same neurotransmitters as dissociative or opioids, at least not in any way close enough to categorize it as a dissociative or opioid. There's absolutely nothing enjoyable about tranq unless you like feeling unbelievably tired and nodding off constantly with no euphoria. The fent adds euphoria to it I guess, but even then, most people would rather be doing pure heroin, oxycodone, oxymorphone, or hydromorphone than fentanyl. Fentanyl is less enjoyable and lasts 1/4 or less as long as H. After the government made it nearly impossible to be prescribed opioids these counterfeit pills pressed with unknown doses of fent started appearing. Addiction, use, and deaths have only increased. So once again the government has made the problem worse. Learning nothing from alcohol prohibition or the decades of this failed drug war.

The information she gave is correct but "French Homie" is also correct - she is not a Fellow of the American Academy of Dermatology (FAAD) and is not certified by the American Board of Dermatology. MDs, and the government, take false representations of being a medical doctor pretty seriously.

How was the Cajun restaurant?

That makes sense. I'm a dermatologist so I thought her interview seemed off like she was bullshitting. I also noticed she said "vice president of medical affairs for a small dermatology company"

Theres 420 likes i cant like even if i like this
French Homie Guam
Super misleading to refer to a registered dietitian with a PhD in Nutritional Science and Biomolecular Chemistry as a Dermatologist
French Homie Guam
She is not a medical doctor or physician. She is a registered dietitian with a PhD in nutritional science and biomolec chemistry.

You’ve got to post this on all platforms as a public service.

She gave a great interview! So good at explaining!

There are some additional small things that other people pointed out, for example although xylazine and ketamine are both anesthetics, they work on completely different mechanisms.
Adrian Millman
Because there is a miscommunication between the two. The chronic ulcers that these people suffer with is not necrotizing fasciitis. Nec fasc is an acute, rapidly progressive, deep tissue infection. IV drug users are also at an increased risk for nec fasc, which is where the confusion stems from. But conflating the two conditions is medically incorrect, as they have very different pathophysiology, different treatments, and wildly different prognoses.
Michael
Nah no way man, doesn’t matter who’s in power drugs are getting into the country. Addiction is more of a social and economic issue. The problems mainly lay within the disproportion of wealth in your country and the lack of access to a lot of basic needs due to cost and privatisation. In Australia we have a lot of wealth inequality too, not close to you guys but a key factor is the accessibility of services that prevent drug addiction
Michael
Idk smoking weed almost ruined my life. I was smoking it all the time and doing nothing but sitting at home. I’ve been a fairly heavier user of party drugs like coke, MD, ketta, benzo’s, and psychs speed but have never been close to addiction
Jason Martin
for lack of better phrasing... you sound dumb
Jason Martin
Why would a professional in the frontier of a specific field and years and years of experience along with a doctorate in medical studies need fact checking?
BigMoney
Psychedelics are pretty sick aswell.
J Borucki
Wrong PR invented the use of tranq in dope mix to raise profits about 10 years ago then soon started selling each separate $2 tranq $3 dope ( heroin&or Fet) due to demand, then soon exported idea to philly because large PR population in kensington. Source - vice news- kensington resident- check facts dont spread misinformation- peace-love-happiness
Adrian Millman
Needs medical fact checking. Love the content, though.

just to hop on this bandwagon - there are a ton of other providers with real credentials and on-the-ground experience who you could have spoken with for this interview. The wound care nurses at Prevention Point are a wealth of knowledge on the topic and have been doing a lot of work to propagate best practices in the face of increased xylazine prevalence. Ditto, her mention of people not staying in the hospital or keeping appointments in order to chase highs papers over a HUGE problem in the community, which is provider stigma and inadequate withdrawal management in the in-patient setting. Xylazine and fentanyl withdrawal play a big role there and if you had asked ANYONE on the Ave, they would have told you the same.
RA
Tranq is not a thing in Puerto Rico, where did she get this info?
Steven Vick
What are YOU personally doing about the issue then? People like YOU are making excuses for fascism and won’t even get your hands dirty.

Love the expert interviews

This is insane, like fuck drugs man, weed only. I feel for these people, when I visited San Francisco I saw a dude just dead from an overdose in front of a nice Cajun restaurant.

thanks for this clarification andrew! excellent as always

Man I could count her nose hairs in some of those zoom ins

Sure thing. I do work developing new NMDAR antagonists (mostly for pain indications but also very interested in use in depression) as well as studying their pharmacological effects (this includes ketamine) for basic science but also to improve future drug development projects. So med chem and pharmacology. As to your thinking - my take is like most substances, ketamine has many potential uses some of them easy to call therapeutic, others easier to call non-therapeutic, and other harder to place uses which may fall in between. But there is no question ketamine has demonstrated powerful rapid acting antidepressant effects in many randomized controlled clinical trials. Spravato (a nasal spray which contains an enantiomer of ketamine called esketamine) is FDA approved as an add-on for TRD and MDD associated with acute suicidal ideation). There are also smaller but still relevant clinical studies supporting efficacy in suicidal ideation (the thoughts themselves in this case), pain, substance use disorder and more.

Fascinating. I had no clue the cartels were involved. Seems like this issue will not be resolved anytime soon unless the USA rages war on the cartels. Which is impossible bc of their cia ties. The root issue is corrupt govt. the only solution of hope is trump 2024

I very much like this approach of rounding out your "on the street" journalism with great 1 on 1 interviews of qualified individuals. You're doing a great job of coverage on the tough issues. No notes
Sybren Brouwer
Very interesting, thanks for sharing! For my own curiosity, what about ketamine are you studying? Should I be thinking ketamine as a treatment or something else?
Chums McGuffey
Really enjoy the relevant interviews with subject matter experts between parts in series similar to your interview with professor James Gelvin for the Israel rally and Palestine rally videos. Great and insightful videos as always.

Pharmacologist/medicinal chemist here. Interesting interview. One minor thing to point out - while both ketamine and xylazine are used as anesthetics the mechanism of action (how they work) is very different. Ketamine acts (for the most part) as an antagonist at the NMDA receptor, a type of glutamate ion channel (it acts as a channel blocker). Other NMDAR antagonists include PCP and dextromethorphan (DXM). Xylazine acts predominantly as an agonist at the alpha2 adrenergic receptor (similar to clonidine). This reduces the presynaptic release of norepinephrine and has a sedating/anesthetic effect. I study ketamine for a living so felt obligated to point that out! Also I think what is really "evil" is that the government will not do its job and regulate these markets and offer people evidence based medical services to help reduce harm and get them help. While I understand the sentiment, the notion that banning a substance would have any real impact on availability is absurd, aside from MAYBE shifting the market to other similarly acting drugs - usually less well tested and understood. There is clearly a demand for "unapproved" psychoactive drugs that is going no where and prohibition has been an absolute dumpster fire of failure that exacerbates all the inherent harms and creates many new ones. Frustrating to see the largely avoidable loss of life and harm day after day.
Matthias Bernsdorf
Yeah this lady doesn’t seem to have the best understanding
Ben M
No there are actually tons of people willing but no one has the power. People like you will make excuses until the end of time and never effect any positive change in your life.

I love Ketamine <3

I used to perform veterinary surgeries as a laboratory neuroscientist, and we used xylazine in our anesthetic cocktail. You have to be SO careful, especially if you need to do multiple surgeries (so multiple administrations of xylazine per animal). It's tragic but unsurprising to see it gaining popularity on the streets. Ps. The lab coat + sweat pants + comfy shoes is so legit, that's the outfit of a scientist with her priorities straight.

Ketamine is pretty fun

cant tell if youre really good at sounding as if you are what you say, or you are actually an emergency physician. Either way, very well written sir, in all aspects.
Sky Mercure
Andrew, Thank you for what you do. I'm so grateful there's a newscaster I can trust out there.
Ben Kitchel
lmaoo you saw them too huh
Greg Doctor
Emergency physician here. This approaches medical accuracy and is an improvement on the initial segment, but again misses the mark. What many people who use xylazine intramuscularly (shooting into muscle on accident or on purpose) or intravenously develop are necrotic ulcers. This is NOT necrotizing fasciitis. However, a necrotic ulcer likely increases the risk of necrotizing fasciitis, but it is a VERY different entity. A necrotic ulcer simply means there is dead tissue. You can have dead tissue without an infection. Necrotizing fasciitis is an infection caused by bacteria that ultimately invade the fascial tissue and can spread very rapidly. Necrotizing fasciitis is only diagnosed by surgery and many of these patients will die in a matter of hours to days despite aggressive surgical resection, heavy duty antibiotics, and ICU care. As other viewers pointed out, Dr. Cartwright is indeed a doctor of philosophy and not a medical doctor. I do not blame her for not having a full understanding of the pathophysiology of necrotizing fasciitis.
Baba Singh
Andrew, I work in pain management. Your interview was spot on for all the topics covered. Dr. Cartwright knows what she is talking about. Especially about infection not being contained to the injection site. Sepsis spreads through the blood because it's basically an inflammatory infection of the blood. Your coverage is accurate and gives a lot of insight for any harm reduction educator. One of the reasons people are getting these infections is that they believe it's only missing a vein that causes it. That's heroin user thinking. Tranq causes this stuff by dint of the drug itself and not by its route of administration. If your viewers want more info than that they can go to med school, lol.
Miccalo
You're wrong to think wishful thinking, confirmation bias, social polarization, and similar evils only affect half of the two party system.

I wonder if we would ever start paying people to stay sober, or is it more profitable for the medical companies to keep them high
Chris Pyle
Ahh but he did it in a hot tub. And he was on an opioid. He died from drowning. Hot tub is not the best place to solo a bag of sketttaaami

Sadder that republicans aren’t in homes for the mentally ill

Wasn’t healthy for Matthew Perry who died from a massive overdose a month ago
Kevin McDonald
She covered the topic well and presented complex topics in an easy to understand way, while also covering the breadth of different subtopics in this interview. I don't care if she's board certified or not, she satisfied the information gap that this interview was looking to fill. Great find AC
Miccalo
Good callout tbh. Thanks for the post and fact-checking in general.
Miccalo
Sad world, everyone's on drugs. Not judging, I want to help so all don't feel the need to escape with them. Sadly, cliche as it is, every addict has to want to find another stress relief method before discussion of drug removal becomes remotely beneficial. I don't see that happening for those whose habits have lowered them to the streets, nevermind Tranq addiction. Outreach required.

WDYM by this- that it was an honest mistake or are you saying your patreon community is being too scrutinizing?

Shout out to my main man Ketamine - the healthy alternative
Andrew Matson
Ha I love your style Andrew....that said I would have thought that the subject of the interview would have made sure to provide her CV. Clearly she is in the Pharma game, however she is a doctor similar to the way I am! You should try to find someone from Temple Hospital to interview I would imagine that is ground zero for medical services near Kensington.

In addition to insta, on the merch post he said "P.S - if you bought a zine, they’ll be shipped out this week!!! Just been filming like crazy and haven't had time to sign them all, but they'll def be there by x-mas morning!"
JAKOB
I would’ve loved to see her reaction to you saying you thought ketamine was great in your youth lol!! Good stuff!
Nova
Appreciate the attention to detail this video shows, and am very interested in more similar content. Would be particularly interested in hearing more on this from some of the medical personnel that care for these people more directly, like nurses at the clinics that serve these areas

Appreciate the content! Just wanted to give you a heads up, while the interview was really interesting and Dr Cartwright did a good job explaining some topics, she is not a dermatologist. Just from a quick google search I can see she is a dietician and has a PhD, but this does not qualify her has a medical doctor (MD/DO) that is board certified in dermatology. To get greater detail, information, and insight on the issue, I would look into talking to pain medicine, addiction medicine, and other general physicians dealing with the issue in a true clinical setting.

WE HAVE THE SAME NAME 2nd ever Martina ive seen in the wolrddd

These videos have been awesome. Incredibly sad but important work he's doing! Keep it up!

Why a dermatologist? Also, the acting in the wide shots is rough.

with what resources/who doing the enforcing? there are barely enough ppl willing to wipe a sweet grandmas ass let alone wrangling drug addicts

Its almost weird how these pharmaceutical companies cycle the destruction. And for them to have users in ER with no way of paying is almost a crime to humanity. But the thing I've noticed that is immovable is that you can't change a person who doesn't want to change and won't help themselves.
