I think she may have overdone it with the lungs. :)
I'm going with the cave elf thing, considering the storytelling. Maybe the woman in the flashbacks grabbed a cave elf child or halfbreed, because she believed (maybe rightly) that no one would care about the kid. Her intention? Use her as a host for a Drath she could bind.

Thistle Theory: Thistle's form is naturally abhorrent to all light, which is why she appears to be completely black except for her eyes beneath her hood, and why she is only visible as a silhouette here.
Thistle is a form of tiefling? Or this universes equivalent anyway. A lot better than I feared ☺ was halfway expecting her to have that twisted around face that the demon thingies had.
