So I used to think hands were the absolutely worst thing to sculpt. Turns out, it is shoes. I hate shoes. I flattened a cube for the bottom, and added a tapered cylinder to the top. After dynameshing them together I started to define the shape more. What I have found most helpful is to get the front ridge of the shoe looking nice. This prevents the shoe from looking like a sock. Alt+DamStandard brush works well for this!
I also use a ridiculous amount of clay polish.
