The dragon head on the chest is not just his body, that is literally his Gavv. His Gavv is so massive that it covers his entire upper body.

I don't think they have any plans to bring in a 4th Rider (aka Sachika) she's a regular civilian human, plus you'd need the granute organ surgery thing Hanto got to get the ability to henshin
