Lost Rain

The gasps of pain and all that were indeed intentional to help blend in with “normal” people. I’ll add some more details to make it a bit more apparent. Something to keep in mind is that Shiro, for the most part, learned Cues by playing Poker. She’s much better at telling if someone is bluffing or not than reading emotion. The emotion she picked up from the doctor was more the clinical curiosity, ‘run another test’ kind, which isn’t necessarily a great thing since she has so much to hide. I’ll rewrite that bit about the doctor being creepy too so it can show a bit more clearly.

Forgot about the Fox Paw stuff, yeah she definitely shouldn't be wincing at the mundane pain she's experiencing. She is currently impersonating an officer, so I guess the deception is something she's capable of, but there isn't really an intent or a reason to described from her POV. I think I nagged at the author enough to get a little tweak to how the cues perk works/manifests, this may be me basing complaints on my "head canon" rather than the actual writing. Still, I think she shouldn't get a creepy vibe from the doctor who self described as thinking "her tests came back weird and I don't know why, but gosh golly she's so cute I just want the best for her". Actually, you're right that could come off as creepy, maybe I'm reading too much into literally one line in Shiro's pov.

Another note is that her experience in the Fox's realm training the Fox Paw did supposedly desensitize her to pain quite well. I can imagine she vocalized the pain intentionally though, to better play her part in front of the doctor. As for her Cues, I believe it was previously shown it's not a perfect perk, and the cues people give off are not always clear, but while the doctor might not have been among the people skilled at intentionally obfuscating her clues, she was also dealing with some unusual and medically interesting results that could have made her look... uncomfortably interested.

"Never in her life would Holly have thought she’d say this, but she almost felt bad for the pain she caused her newest patient." This is weird to me for two reasons: a street kid not being a little desensitized to pain, and a doctor not feeling bad about causing pain. No other time Shiro has performed field medicine on herself, has it seemed like she was vocalizing her pain. Plus, I know she's more the crafty sneaky scavenger type, but surely her rough upbringing (and close relationship with another doctor) makes me think she's suffered more than just a few scraped knees. Every other part of the doctor's internal monologue makes her seem like a genuinely compassionate doctor, is this line meant to imply she's actually a sadist? Oh, also, I think Shiro being creeped out by the stares of Holly might be another example of her "Cues" perk not holding up to its description. What she should have picked up on is sympathy and regret and the doc thinking Shiro is cute, not anything too creepy. It seems like Shiro just senses some bad vibes, when in actuality she should know what every eye twitch means. Honestly, I feel like Cues is gonna cause problems with the characterization you're going for, cuz it's gonna be tough having a naive and unsocialized gutter kid being oblivious who can ALSO immediately categorize every facial expression she sees.
